PAYMENT AND BILLING POLICIES
Cyrex Laboratories is not contracted with any health insurance providers and does not offer direct insurance or Medicare billing. Patients are financially responsible for services provided by Cyrex. Patients may themselves submit their detailed Cyrex receipt to their insurance company for reimbursement, if reimbursement is offered by their insurance company. Full payment must be received for services before samples can be processed by Cyrex Laboratories.
DIRECT PRACTITIONER BILLING
Practitioners who wish to have patients pay them directly, and be billed directly for Cyrex services, may do so using Cyrex’s online ordering system. Account billing is subject to approval and due upon receipt of invoice. Practitioner account balances over 30 days are subject to finance and service fees and charges.
Payment for services must be received before samples can be processed by Cyrex Laboratories. Payment may be made:
1) From the practitioner’s office during the online ordering proces
a) On practitioner account (upon approval)
b) On practitioner credit card
c) On patient credit card
2) By patient from home after the order is placed:
a) Online at www.CyrexLabs.com
b) By telephone at (877) 77-CYREX (772-9739)
Professional courtesy testing is prohibited by federal law (CLIA).
