PHLEBOTOMY SCHEDULING INSTRUCTIONS                                                    Updated 7/1/2011

Cyrex Laboratories is proud to offer to your patients the convenience and accessibility to hundreds of lab draw centers across the nation. In addition, Home Health Service scheduling is available for those who need a home or office draw.

Please be sure to read and follow these instructions thoroughly and distribute to your staff. Failure to follow these instructions will result in your patients not being serviced at the lab.

Please remember to do the following when ordering the lab tests.

A. Enter – Patient’s email address


This information is required for phlebotomy scheduling confirmations. If your patient has no email address, use an alternate email address (i.e. your office email).

B. Sign and print – Requisition form (2 Pages)
C. Give to patient – Signed requisition form (2 Pages) & specimen collection kit
D. Instruct patient to schedule - Blood draw by doing the following:


1. Click - “Schedule a blood draw” on patient portal of the Cyrex website.
2. Enter – Requisition ID and Confirmation Code
3. Choose – Standard clinics or Home Health Services ($44.50 Service charge applies)
4. Print – Appointment confirmation (a copy will also be emailed to the patient)


E. Patient needs to take following to Phlebotomy appointment:


1. Printed appointment confirmation
2. Signed Cyrex test requisition form
3. Specimen collection kit
4. Patient collected specimen only if patient is having both saliva and blood tested.


*To avoid wait times at the lab, encourage your patient to call the lab beforehand to check on the wait times (confirmation email contains lab contact information).

Instructions on Scheduling Home Health Services

• A phlebotomist can be scheduled to come to a patient’s home or place of employment.
• Home Health Services can be scheduled by any patient, online or by calling Cyrex, for a fee of $44.50.
• Home Health Services must be paid for at the time of scheduling.

Note: Patients in remote areas may incur additional service fees.

Due to circumstances beyond our control, pediatric blood draws are not included under Cyrex’s contracted phlebotomy. All patients 15 years of age and younger are included in the pediatric group. Pediatric blood draws must be scheduled by the parents at a laboratory of their choosing, and must be paid for by the parent at the time of service. Cyrex encourages that patients check their insurance for coverage before scheduling.

Non-contracted labs are not covered in Cyrex’s fees and services. No exceptions. Phlebotomy services at non-contracted facilities are the responsibility of the patient.
